Waiver Release Form
Cancellation/Refund Policy and WaiverYou may cancel your registration and receive a full refund if we receive notice at least one week before the camp starts. Otherwise a club credit will be given to be applied to a future camp. Once a camp has started no refunds, club credit, or makeups will be issued.
I have read and agree to the cancellation and refund policy.
Photographs: Pickleball West Coast is granted permission to use group or individual photographs, photo images, video or other recordings taken during class for publicity or promotional purposes. I hereby grant Pickleball West Coast the right, consent, and permission to use and publish my name, image, photograph, voice, video, appearance and likeness by any means. I hereby waive my right, if any, to any compensation for said use of any such works, images and/or materials. Studio Waiver and Release of Liability Ability to engage in pickleball activities and assumption of the risk.Pickleball West Coast takes all reasonable precautions to reduce risk and provide safe, healthy, and enjoyable experiences. I warrant that my child or ward is able to follow directions for all activities in pickleball camp. I acknowledge that risks from participation in class activities exist and that I have allowed my child to attend pickleball activities knowing these risks and their possible consequences including personal injury.Waiver and Release of Liability: As a parent or guardian of my child or ward, I, on behalf of myself and my child or ward, agree that I will not hold Pickleball West Coast liable for any personal injury, property damage or loss of insurance. I hereby release, discharge and agree to indemnify, defend and hold harmless Pickleball West Coast and Shellton Jean Baptiste, and each of their volunteers, staff members, agents, representatives, owners, officers, directors, and employees (the “Released Parties”), from all claims, actions, demands, and causes of action, of whatever kind or character that I or my child or ward may have or claim to have against the Released Parties, for any and all injuries, harm, damages (actual and punitive), penalties, costs, losses, expenses, attorney’s fees, and/or other liability or detriment arising out of or in any way related to attendee’s participation in or attendance at any class conducted by Pickleball West Coast. The undersigned parent and natural guardian does hereby represent that he/she is, in fact, acting in such capacity, has consented to his/her child or ward’s presence and participation in activities at Pickleball West Coast and has agreed individually and on behalf of the child or ward, to the terms of the Club Permission and Waiver Form. The undersigned parent or guardian further agrees to save and hold harmless and indemnify each and all of the Released Parties from all liability, loss, cost, claim or damage whatsoever which may be imposed upon said parties because of any defect in or lack of such capacity to so act and release said parties on behalf of the minor and the parents or legal guardian.
